BARP, flew p2p! Anesthetized under 3% iso for repeat rads. Dr. G opted to extract pellet from elbow with no complications. Sutured site with 4-0 PDS II. Recovery a bit prolonged but uneventful. Decided to move bird somewhere smaller while wing stays bandaged. Recheck in a few days.

Bird BAR and flying in Mew. Small so can't assess flight well but not really labored. Minimal R wing droop but elbow still appears thickened. Incisions are scabbed.

Anesthetized w/ Isoflurane for radiographs.
Pellet still in same place. Soft tissue swelling. Concerning increase in opacity of distal humerus.
ROM in elbow slightly decreased.
Since overall he is doing well and flying will give another week to recuperate from previous surgery. However, if suspicion of bony change continues will have to go back in and be more aggressive.
Change antibiotics to Enrofloxacin and Clindamycin for bony coverage.
Recheck outside and radiographs in 1 week. Dr Gardner

Bird has been getting regular PT - still very reactive to manipulation of the elbow. Opted to anesthetize to repeat radiographs and consider attempt at surgical removal.

Wound on back is healed.

P: Bandage change every 3-4 days. Once wound is healed consider leaving unbandaged if bird does not have wing droop. Will recheck 1/10. Perhaps the pellet will work its way more to the surface. Dr Gardner

Surgery time - 0 minutes

Anesthetized and performed VD and skyline radiographs to pin-point
the location of the pellet. Radiographs showed the pellet on
the medial aspect of the elbow right at the joint. Placed needles
in an attempt to further pin-point the location - needles were
further able to isolate area, but pellet seems to be right under
the ulnar vein where it courses over the elbow. 
Brought the patient to the OR with the needles in place. Intubated
and prepped the area. Incised just distal to the joint and retracted
the vessel aside to tunnel under it and explore the tissues in
that area. 
Explored pocket under the skin in all directions with no success.
Gently dissected some of the tendon in that area to explore beneath,
again with no success. 
Before closing the incision, brought back to the radiology suite
to make sure it hadn't moved. Radiographs showed the pellet in
the area of my incision, deep to the skin and likely embedded
in muscle. 
Brought bird back up to surgery and again explored the area,
deeper into the muscle in the area but was still not able to
find the pellet. I was concerned that if I explored any further
I would cause more damage than the pellet was currently causing.

Closed incision in a simple interrupted pattern w/ 4-0 PDS. Turned
the bird over and briefly explored the dorsal wing, thinking
that if I tunneled through the original wound I might be able
to follow the tract of the pellet. However, was also unsuccessful
so closed w/ a single suture. 
Bandaged w/ padding. Bird recovered quickly. Dr Gardner

Bird is BAR.
Anesthetized for radiographs - H-view, right lateral, skyline of R wing.
Pellet in elbow looks as though it is on the medial aspect of the elbow joint, right where the humerus meets the radius/ ulna. On the other side from the wound.
The pellet in the chest appears to be embedded in muscle in front of the humerus.
Cleaned the elbow and looked closely at dorsal and lateral aspect. Could not see anything that appeared to be the pellet. Suspect it may be buried in the tissue of the elbow joint, perhaps under the tendon that courses across the ventral elbow. This area is where the major vessels run as well, so I did not feel compelled to deeply explore that area. Hopefully the swelling will go down and we will be able to eventually palpate the pellet.
Felt a small scab on the chest - there is a tiny area of inflammation, may be associated with the pellet there but I do not see or feel the pellet.
Cleaned wound on back and closed with a single suture.
Applied Silver gel to elbow wound and bandaged with a figure-8.
Bandage change every other day for now. If Tracey is here 12/24, take a look at it. If not, I will recheck 12/31. Dr Gardner

Bird was on perch upon arrival, placed on perch when returned to kennel, but he stood on the ground with his L leg, leaned against perch and was flaring defensively.

QAR, puffed up & mouth open, lying in donut this morning. 44g LO removed. At time of tx was more BAR and perched. Crouched down while on perch. Wound on back discovered over pellet site, feathers sticking out of it. Opened wound to clean & get feathers out of - lots of feather strands came out, very deep. Cleaned with chlorhex and covered with ssd/telfa/tega. Did not disturb elbow wound for now. Set back up in kennel, stood on papers
Note to start antibiotics - which? needed? scheduled for Dr G to look at/decide tomorrow.
